چکیده

Most heart attacks and ischaemic strokes are caused by complicated atheroma, usually compounded by thrombosis suddenly reducing blood flow in a critical artery. Extensive evidence suggests that this atheroma silently builds up over decades. Thus, early atheroma streaks were seen in classical autopsy studies on Korean war casualties and teenage traffic fatalities. Hence, the emerging paradigm describing the temporal relationship between risk factor change and the corresponding change in CVD mortality. The time scales for this paradigm are generally perceived in terms of decades. Thus, Rose thought the ‘incubation period’ was a decade or more, whereas Law et al. proposed a three-decade lag time to explain the paradoxically low French CVD rates. Reversing the CVD process following reductions in major cardiovascular risk factors has therefore also been assumed to require decades. However, this is wrong.

Dynamics of cardiovascular and all-cause mortality in Western and Eastern Europe between 1970 and 2000.

AIMS Important changes in cardiovascular and all-cause mortality rates are occurring in Western and Eastern Europe, each with their own dynamics. Differences in trends will be analysed and possible causes are discussed. METHODS AND RESULTS Mortality data for cardiovascular and all-cause mortality rates from different countries were obtained from WHO and were analysed for the period 1970-2000....

Cardiovascular prevention: lifestyle and statins--competitors or companions?

Favourable lifestyles promote cardiovascular protection. Exercise can induce beneficial changes in the genome that decrease low-density lipoprotein cholesterol (LDL-C) and increase anti-inflammatory markers. The Mediterranean dietary pattern, fortified by nuts, while not reducing weight, reduces mortality. Lifestyle changes combined with statin therapy provide potent protection against coronary...

Nutrition and health.

Important differences in all-causes mortality and in cardiovascular mortality exist between populations and they cannot be explained by differences in the level of medical care. During the last 20 years important changes have occurred in the level of cardiovascular mortality with an increase in Hungary and a decrease in Japan. These rapid changes in mortality cannot be explained by genetic fact...

Ecological study of reasons for sharp decline in mortality from ischaemic heart disease in Poland since 1991.

OBJECTIVE To investigate the reasons for the decline in deaths attributed to ischaemic heart disease in Poland since 1991 after two decades of rising rates. DESIGN Recent changes in mortality were measured as percentage deviations in 1994 from rates predicted by extrapolation of sex and age specific death rates for 1980-91 for diseases of the circulatory system and selected other categories. ...
